IIRC, the two choices vary the amount of objects preloaded by the game. The tradeoff is that in Extreme, the game uses a ton of memory but there will be no stuttering as it loads objects, whereas with High there may be some occasional stuttering but the memory demands will be less.

EVE isn't a violent game, its more of a strategic space ship battle game, you can fly around on this huge universe and pirate peoples shit and blow stuff up, which is fun! It is bit confusing at first but no gore, gruesomeness or vulgar language that you may disapprove of. Though be weary it is a MMO so other people may be using bad language but there is a filter you can turn on for foul language.
